Your First Visit

For your first visit please bring the following with you:

• Photo ID
• Insurance Card
• Disc with your X-ray/MRI/CT Scan images from outside clinic/hospital
  • We do not have access to imaging from non-VCMC affiliated hospitals or clinics (St.Johns, CMH, Ojai Valley, Clinicas, Los Robles, Grossman Imaging, Simi Valley, local Urgent Care etc.) If your imaging was performed elsewhere it is YOUR responsibility to obtain and provide copies of imaging at the time of your initial appointment.
    • If you fail to bring your most recent images with you your appointment may be cancelled/rescheduled.
    • If your insurance authorizes, a repeat X-ray may be required.
• Hospital Discharge Summary (if recently discharged or seen in the Emergency Dept)
• Any Work/School Excuse Forms that require completion/Physician signature

**Parking is limited. Please plan to arrive early to the clinic to allow ample time for parking and registration for your scheduled appointment. **

Please note that your appointment time is NOT the time you should expect to see the Physician or Physician Assistant.Please take into consideration time needed for registration and nurse assessment (vitals). If you require splint/cast removal and/or x-rays this will be performed PRIOR to being seen.
